Well, he'd been under the influence of the Ring since the day Deagol found it and Smeagol slew his cousin in order to get it for himself. I suspect that it took a fairly long while for their grandmother to bring herself to accept that for the sake of the rest of the family Smeagol needed to leave and stay away.
